Real-Time Biometric Streams

Instead of relying on patients to log their vitals manually, MIH programs are distributing pre-configured, cellular-connected devices:

  • LTE Weight Scales: Crucial for CHF patients, weight changes are transmitted instantly, flagging fluid retention before physical symptoms occur.
  • Continuous Patch ECGs: Wearable sensors monitor heart rate variability and arrhythmia events, transmitting alerts directly to the MIH dispatch center.
  • Smart Pulse Oximeters: Provides daily oxygen saturation readings for COPD patients, identifying respiratory decline early.

Trigger-Based Dispatch

Rather than running scheduled check-ins, MIH dispatchers monitor an alert board. When a patient's vitals cross predefined thresholds, the system flags the profile and prompts the dispatcher to route a community paramedic to the residence.

The Outcome

By replacing scheduled visits with trigger-based interventions, MIH programs are reporting meaningful reductions in 30-day hospital readmissions — a metric tied directly to CMS penalty exposure — though the magnitude varies widely by program design and patient population.
