Prehospital POCUS: Ultra-Portable Ultrasound in the Field

Handheld ultrasound probes are transitioning from emergency departments to field vehicles, helping medics identify internal hemorrhage in minutes.

Shifting the Diagnostics Boundary

In high-acuity trauma cases, identifying internal bleeding early is the difference between survival and mortality. Point-of-Care Ultrasound (POCUS) has been a hospital standard for decades, but large cart-based machines were logistically impossible to carry in ambulances.

The development of handheld, ultra-portable ultrasound probes (like the Butterfly iQ+ or Vscan Air) is bringing diagnostic-level imaging directly to the field.

Performing the eFAST Exam on the Street

Medics use handheld probes connected to clinical tablets to run the Extended Focused Assessment with Sonography for Trauma (eFAST) exam:

  • Internal Bleeding: Identifies free fluid in the peritoneal, pleural, or pericardial spaces within two minutes.
  • Pneumothorax Detection: Confirms lung sliding, allowing crews to perform needle decompressions with confidence before transport.
  • Cardiac Activity: Evaluates cardiac standstill during resuscitation efforts, giving medical directors objective data to guide field termination protocols.

Tele-Ultrasound Over Wireless Networks

The latest generation of POCUS devices allows real-time streaming of ultrasound video feeds to the receiving trauma center. While the paramedic is in transit, the trauma surgeon can view the ultrasound display, prepare the operating room, and bypass the emergency department entirely upon arrival.

The Implementation Roadmap

While POCUS is highly effective, implementation requires comprehensive training programs, quality assurance audits by EMS coordinators, and strict image storage compliance.
