Dr. Peter Safar: The Three Phases of Cardiopulmonary Resuscitation

Modern CPR did not emerge overnight. Meet the pioneer who combined airway control, chest compressions, and emergency education to save millions.

The Architect of the Life-Saving Alphabet

Every firefighter and EMT learns the ABCs of resuscitation: Airway, Breathing, Circulation. This systematic approach feels like common sense today, but before the work of Dr. Peter Safar in the late 1950s and 1960s, there was no unified method to revive a stopped heart.

Dr. Safar, working in Baltimore and Pittsburgh, consolidated separate medical discoveries into the three-phase framework of Cardiopulmonary Resuscitation (CPR).

Phase I: Basic Life Support (BLS)

Dr. Safar's first breakthrough was demonstrating that the head-tilt/chin-lift maneuver reliably opened the airway of an unconscious patient. He combined this with mouth-to-mouth ventilation and closed-chest cardiac compressions, proving that laypeople could maintain oxygenation and blood circulation until doctors arrived.

Phase II: Advanced Life Support (ALS)

Working with early EMS teams (including Pittsburgh's Freedom House), Dr. Safar established protocols for prehospital intubation, intravenous drug administration (such as epinephrine), and portable cardiac defibrillation, moving intensive care out of the hospital and onto the streets.

Phase III: Prolonged Life Support (PLS)

Safar's vision extended past the initial return of spontaneous circulation (ROSC). He pioneered the study of cerebral resuscitation, researching how hypothermia protocols and blood pressure optimization protect the brain from neurological injury after cardiac arrest.

An Enduring Legacy

Dr. Safar's legacy is carried in every ambulance call, CPR class, and resuscitation attempt performed by first responders worldwide.
