The fire is out, but the carcinogens you carried off the scene are still working. Gross decon on the lawn and a clean-cab discipline are how we keep that contamination off our skin and out of the rig.
The threat doesn't end at "fire's out"
In July 2022, the International Agency for Research on Cancer moved occupational firefighting to Group 1 — carcinogenic to humans, the same tier as tobacco smoke and benzene. That wasn't a press release to frame on the wall. It was IARC's working group reviewing 52 cohort and case-control studies and concluding there is *sufficient* evidence in humans for mesothelioma and bladder cancer, with limited evidence for colon, prostate, and testicular cancers, melanoma, and non-Hodgkin lymphoma.
The contamination vector is mechanical and chemical. Products of combustion — polycyclic aromatic hydrocarbons, benzene, formaldehyde — condense onto our gear as soot and oily residue. Heat opens skin pores and drives dermal absorption; the neck, jaw, and groin are the worst offenders because the skin is thin and the gear seals are weakest. Then we throw that contaminated shell in the cab, breathe the off-gassing on the ride back, and store it next to where we eat and sleep.
Decon is a spectrum, not a single step
There are three distinct levels, and confusing them gets crews hurt. Gross decon happens on scene, before you take the gear off. Technical decon is the deep wash. The clean-cab concept is the systems discipline that keeps the other two from being wasted effort.

Gross decon is the highest-yield, lowest-cost intervention we have. A bucket of water, Dawn, and a soft brush — or a hose stream on a fog pattern — gets the bulk of the soot off *before* you doff the SCBA. Stay on air until your face and neck are wiped down with decon wipes. That's the window where PAHs are still on the outside of the gear instead of in your bloodstream.
The clean-cab movement and your second hood
Clean cab means the contaminated turnout shell never enters the crew compartment. Gear lives in exterior, sealed compartments; the cab gets hard, wipeable surfaces instead of fabric seats that soak up combustion residue. Pair that with a second-hood swap — particulate-blocking hoods are cheap, and the hood sits against the carotid skin that absorbs the most. Carry a spare, bag the dirty one on scene, and don the clean one. Same logic drives spare-gear programs: if your set is soaked in products of combustion, you should not be wearing it to the next call. This dovetails with the toxicology of smoke itself — the same HCN and PAH-laden smoke that poisons your patient is what's plating out on your shell.
The PFAS problem baked into the barrier
Here's the uncomfortable part. The moisture barrier that keeps you dry during suppression has historically relied on per- and polyfluoroalkyl substances (PFAS). NFPA's requirement that turnout be effectively moisture-proof made PFAS chemistry close to a default. So the gear protecting us from one carcinogen has been a reservoir of another, and it's right against our skin under thermal load.
The standards caught up. NFPA 1851 — Selection, Care, and Maintenance of Protective Ensembles — has now been consolidated with the SCBA standard into NFPA 1850, effective September 9, 2025. The new edition pushes a non-fluorinated durable water repellent (silicone- or wax-based DWR) and establishes optional verification procedures specifically aimed at removing PFAS during cleaning. It also broadened the verification framework for confirming contaminants are actually being removed, not just smeared around. That matters because an annual advanced cleaning that hydro-tests the moisture barrier is mandated, and you want that wash to be subtracting carcinogens, not redistributing them.
None of this is exotic. Soap, water, exterior gear storage, a spare hood, and a wash policy that follows the standard. The same situational-awareness discipline that drives adoption of in-mask thermal HUDs belongs on the back end of the call too — the job isn't done when the line is shut down. It's done when the contamination is off you.
